    解説

    虚血性脳卒中(ischemic stroke)の大多数は血栓による脳動脈の閉塞により生じる。虚血性脳卒中はアテローム血栓性脳梗塞、心原性脳塞栓症、ラクナ梗塞、その他に分類される。虚血性脳卒中のうち非心原性虚血性脳卒中にはアテローム血栓性脳梗塞とラクナ梗塞が含まれるが、これらの虚血性脳卒中の再発予防には脳動脈を閉塞する血小板血栓の形成を抑制する抗血小板療法の適応がある1)2)。抗血小板療法は虚血性脳卒中の再発を20~30%低減する効果がある3)。日本で非心原性虚血性脳卒中患者に用いられている主な抗血小板薬にはアスピリン、クロピドグレル、シロスタゾールがある1)2)アスピリンは医療経済効果に最も優れている抗血小板薬であるが、頭蓋内出血と消化管出血のリスクが高くなることに注意が必要である4)。クロピドグレルはアスピリンより再発予防効果がやや優れているが、まれながら肝機能障害や血球減少症をきたすことがあり、遺伝子多型に関連した薬剤抵抗性の存在が指摘されている5)6)シロスタゾールアスピリンやクロピドグレルより出血リスクが少なく、特にラクナ梗塞ではアスピリンより脳出血のリスクが少ないが、副作用として頭痛や頻脈の頻度が高い7)8)。発症後早期の非心原性虚血性脳卒中にはアスピリンとクロピドグレルの二剤併用療法(DAPT)が有効であるとの報告がある9)が、長期のDAPTは出血リスクが大きくなるので推奨できない10)11)。高リスクの虚血性脳卒中患者の再発予防にはシロスタゾールを用いたDAPTがアスピリンやクロピドグレルよりも有効であり、出血リスクも増加しない12)。虚血性脳卒中患者に抗血小板療法を行う場合には脳出血のリスクを回避するため血圧の厳格な管理が必要であり、130/80mmHg未満の降圧が推奨されている13)-15)。心原性脳塞栓症の再発予防に抗血小板療法の効果は期待できないので、心原性脳塞栓症には抗凝固療法を行うべきである2)
